Intravenous
- < 1 wk
- < 2000 g: 10 mg/kg every 12 h
- >2000 g: 15 mg/kg every 12 h
- >1 mo: 10–15 mg/kg every 6 h
- Adult: 15 mg/kg every 12 h or 6.5–8 mg/kg every 6 h
