Dosage

Intravenous

  • < 1 wk
    • < 2000 g: 10 mg/kg every 12 h
    • >2000 g: 15 mg/kg every 12 h
  • 1 wk–1 mo
    • 10 mg/kg every 8 h
  • >1 mo: 10–15 mg/kg every 6 h
  • Adult: 15 mg/kg every 12 h or 6.5–8 mg/kg every 6 h
